I understand that My Wellness My Life (Hereafter referred to as MWML) is set up solely to teach others how to prepare plant-based food without the use of animal fats, dairy or processed food.

This service is also holistic in nature and any one on one consultations are strictly for education and guidance purposes.

I understand that Doreen Germany of MWML is a Natural Health Educator and Counselor.

I fully understand that Doreen Germany of MWML is not a medical doctor and I am not here for medical diagnostic or treatment procedures.

The services provided by Doreen Germany are at all times restricted to education and guidance on the subject of natural health matters intended for general natural health well-being and do not involve the diagnosing, prognosticating, treatment, or prescribing of remedies for the treatment of any diseaseI understand that I may be allergic or sensitive to an item on the menu and it my responsibility to ensure that those foods are not consumed or are avoided.

I understand that in the case of digestive issues, I may develop food sensitivities to a food that is being consumed daily, and it is my responsibility to remove that food from my diet.

I understand that certain foods are known to aggravate certain conditions and it is my responsibility to disclose any conditions that I may have been diagnosed with, in order for my counselor to cross reference any possible food interactions. However I do understand that some issues I may be dealing with may be undiagnosed and MWML is not responsible for any aggravation of an unknown or undisclosed condition that I may be dealing with.

I will not, under any circumstances, cease to take any medications or supplements that my physician has prescribed for me. I understand that only my physician is authorized to do so. I understand that to do so could be detrimental to my health.

I understand that my fee is for the services of MWML & to educate and provide guidance. No refunds will be provided after the first day of the program.

I certify that this agreement is being signed voluntarily and not under any pressure of any kind.

I certify that I am here, on this and any subsequent visit which includes Phone or email appointments, solely on my own behalf and not as an agent for any federal, provincial, municipal or professional agency on a mission of entrapment or investigation.
